Goal: The objectives are to increase knowledge of SIDS risk reduction strategies, including sleep position and roomsharing without bedsharing.

Impact: A 15-minute educational session with small groups of parents is effective in informing parents about the importance of safe sleep position and in changing parent behavior for at least the first 6 months of the infant's life.

Goal: The goal of this program is to provide parents with the necessary skills to improve their parent/child communication and overall family functioning.

Impact: STEP has been implemented in more than 1,000 schools, agencies, churches, and mental health treatment facilities since 1976, reaching more than 4 million parents. Outside the US, STEP has been implemented in Australia, Canada, Germany, Ireland, Mexico, New Zealand, the Philippines, Romania, and South Korea.

Goal: To use tai chi exercise to improve balance and decrease incidence of falls among older adults.

Impact: The program shows that ta chi can significantly improve health-related outcome measures in older adults and such a program can be practically and effectively implemented and maintained in community settings.

Goal: The goal of The Bridge Model of Transitional Care is to help aging adults transition from the hospital back to their homes and communities safely.

Impact: The Bridge Model of Transitional Care can help lower hospital re-admission rates as well as improve primary care engagement 30 days after being discharged from the hospital.

Goal: The CO2 Reduction Plan was officially adopted by the Chula Vista City Council in November 2002 and aims to reduce CO2 emissions to 80% of 1990 levels by the year 2010. Specific action measures include:

- Municipal clean fuel vehicle purchases
- Green power use and public education program
- Telecommuting and telecenters
- Enhanced pedestrian connections and increased housing and employment density near transit
- Bicycle lanes, paths, and routes
- Energy efficient landscaping
- Solar pool heating
- Traffic signal and system upgrades

Goal: GAIN is part of a large-scale, welfare-to-work initiative program operating in every county in California. In L.A. County, the initiative is under the supervision of the Department of Public Social Services. It helps local businesses and employers find and hire quality workers who seek meaningful employment. Prospective workers are participants in the state welfare programs known as California Work Opportunities and Responsibility to Kids (CalWORKS) or General Relief Opportunities for Work (GROW).
