The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

The goal of the study was to prevent STDs in high-risk minority women through three culture-specific small group education and counseling sessions, delivered over time.
Reinfection rates of chlamydia and gonorrhea were significantly lower at each follow-up among participants in the small-group counseling sessions than in the control group. Integration of behavior-change theory with extensive qualitative data collected in target communities enabled the study to create culturally meaningful strategies to promote the recognition of risk and to stimulate motivation to effect personal change.

The goal of the Checkpoints Program is to encourage parental limits on teen driving, and decrease risky teen driving.
The Checkpoints Program increased the rate at which parents placed greater limits on high-risk teen driving conditions and decreased traffic violations among teens.

The goal of this workshop is to build the participants’ self-confidence in their ability to manage their health and maintain active and fulfilling lives despite their chronic disease condition.
In 2016 Derby Health Collaborative hosted six free self-management education courses in partnership with the Derby School District, the Derby Senior Center, the Derby Public Library and the Kansas State Extension and Research Center. Workshops will continue in 2017.

The goal of Color Me Healthy is to promote and encourage physical activity and healthy eating among children ages four and five.
The program has had a positive impact on children's knowledge of and participation in physical activity. Similarly, it has had a positive impact on children's ability to recognize and their willingness to try fruits and vegetables. It has also increased children's fruit/vegetable snack consumption.
